Warrior doesn't give up

 






A man should fight till the end and so will I…

But I'm only human, weary of the battle…

Will my friends be still there?...

In my deepest and darkest despair....will they still care?

Being an unusual child. I never concerned myself with petty things, the trivia, the dregs in life.My tangential connections with friends and relatives left me isolated but growing up in the shadows of an extraordinary mother who helped to wipe out the bad memories and realize the beautiful, prettier side of life. Yes, never been able to complete my studies— and reading my collection of books at night, like that, under the quilt with the overheated reading lamp. Reading all those good lines while suffocating. It was magic


You can never learn from a book by memorizing a book. Real knowledge is understanding the phenomenon. I grew up reading Carl Sagan and I admire science and I always have a question in my mind.

Indians are always showing their true colors although I have never seen any other country.

The medical profession has many self-centered egoistic people that think they know everything and all patients should just shut up and be grateful and any questioning of their ability or judgment is rare... Who am I a mosquito?

Neuroendocrine tumours (NETs) are rare tumours that start in neuroendocrine cells. You might also hear these tumours called neuroendocrine neoplasms (NENs) or neuroendocrine carcinomas (NECs). 


There are a number of different types of NETs. The type you have depends on the type of cell that the cancer started in. 


Most NETs develop slowly over some years. They may not cause symptoms in the early stages. It’s not unusual for people to find that a NET has already spread to another part of the body when they are diagnosed. 



Neuroendocrine cells have traits similar to those of nerve cells and hormone-producing cells. Neuroendocrine tumors are rare and can occur anywhere in the body. Most neuroendocrine tumors occur in the lungs, appendix, small intestine, rectum, and pancreas.

The neuroendocrine system is made up of nerves and gland cells. It makes hormones and releases them into the bloodstream.


Neuro means nerve and endocrine refers to the cells of the endocrine system. The endocrine system is a network of glands and organs in the body that make hormones. It is also called the hormone system.


There are neuroendocrine cells in most organs of the body, including the:


  • food pipe (oesophagus)
  • stomach
  • Lungs
  • small and large bowel
  • pancreas
  • Liver



Neuroendocrine cells have different functions depending on where they are in the body. They control how our bodies work. This includes our growth and development, how we respond to changes such as stress, and many other things. 


For example, neuroendocrine cells of the lung make hormones that control the flow of air and blood in the lungs. And neuroendocrine cells of the gut (digestive system) make hormones to control:



  • the production of digestive juices
  • the muscles that move food through the bowel
  • Where NETs start
NETs can start in different parts of the body. Like all cancers, NETs are named after the place they start growing. For example, a NET that starts in the lung is called a lung NET. This is the primary cancer. If the cancer spreads to another part of the body, it’s called secondary cancer. 


Around 5 out of every 10 NETs (50%) start in the digestive system. This is also called the gastrointestinal (GI) system. It includes the:


  • stomach
  • small and large bowel
  • pancreas
  • back passage (rectum)

Around 2 out of every 10 NETs (20%) start in the lung. NETs can also start in other places such as the:


  • food pipe (oesophagus)
  • appendix
  • Skin
  • prostate
  • womb
  • adrenal, parathyroid, and pituitary glands

NENs show a spectrum of behaviors and this makes their treatment challenging. Some exhibit an indolent, slow growth pattern, while others parallel the more aggressive, rapidly spreading tumors such as small cell lung cancer (SCLC); in between there are neoplasms of intermediate malignant potential. They require urgent aggressive chemotherapy and are associated with markedly shorter survival.  Tumours of small bowel origin tend to have a better prognosis compared to NENs originating in the pancreas. The effect of other factors such as age, race , resectability , performance status, or even marital status has similarly been examined in several publications. Most medical decisions nowadays consider tumor of origin, staging

Brain metastases (BM) are rarely reported in patients with neuroendocrine carcinoma (NEC) of non-lung origin and neuroendocrine tumors (NET) of the gastroenteropancreatic (GEP) or bronchopulmonary system. However, symptomatic brain metastases are associated with dismal prognosis, so early detection and treatment could be advisable.


The incidence of brain metastases for neuroendocrine tumor (NET) is reportedly 1.5~5%, and the origin is usually pulmonary



Why did I have pancreatic hypotenuse lesions? Why were they not followed up? And the subsequent symptoms investigated? Why were there somatostatin receptors and diarrhea such that I lost 20 kilos? " Oh your brain naturally emits somatostatin" and thus misled tons of psychiatric drugs were given and I gained weight.


But it's the goodness that makes the world spin. And we feel the bright sun will bring good tidings as Dr. Hukku identified the cause of severe diarrhea isn't being a Bengali and eating spicy ( which Dr. Ishita Sen said under medical negligence) food and sent me to a medical oncologist. There is positivity similarly there is negativity and both balance the world.


 There had been a couple of regrettable incidents though. Surprisingly cogs and circuits malfunctioned because of errors and time cheated. I haven't forgotten everything of her past,  brother ( God never give such a money minded brother to anyone)living for the sake of money, his spouse assaulting and saying " You Bitch you die right now"...


Every day of suffering, part of which remained in the circuits and when those fateful memories turn up, the electrons in the brain got charged up and misfired and as I start remembering extra Mum channeled those bottomless energy and i endrted writing and spinning out tales, novels and detective stories and poetry and my penchant for figuring out Science never left me.

I jotted them down on my blog.

The desire to time travel and go back to my childhood days expunging certain events that never let her enjoy the hop-skip-jump of those days was a strong attraction .Realizing such a strange tour is out of the question, wrote and wrote and wrote and wrote here I am writing my fall. I slid down but a warrior is born to fight, not to give up.

No money no life

 A son instead of being dutiful towards his parents and ailing sister desires the end of the ailing, helpless and shoves them out of the way, 

A raving egomaniac parent drunk with the influence of wealth, affluence, power withholds what the ailing sibling truly deserves, thus strangling life out bit by bit




Since you ask, most days I cannot remember.

I walk in my clothing, unaware by that voyage.

Then the almost unnameable lust returns.


I never wanted to die. Even when those I loved

died , went away from me, went beyond me. 

My life was never always a question, if for no other reason

Then I wanted to wake up and see what happened next. 


Even then I have nothing against life.

I know well the grass blades you mention,

the furniture you have placed under the sun.



But suicides have a special language.

Like carpenters they want to know which tools.

They never ask why?.


 I have so simply declared myself,

I have been beaten by  the enemy, eaten the enemy,

Have taken on by  his craft, his magic.

Who asked me to drop dead.

Pulling the thread by which I was alive,living jolly breathing, not dead.


In this way, heavy and thoughtful,

warmer than oil or water,

I have rested, drooling at the mouth-hole.


And I continued to want to open like that, like the flowers

who lift their heavy heads as the hills outside the window

flare gold for a moment before they turn

on their sides and bare their creased backs.


Even the cut flowers in a jar of water lift

their soon to be dead heads and open

their eyes, even they want a few more sips,

to dwell here, in paradise, a few days longer.



I did not think of my body at needle point pain

Even the leftover will is  gone.

Suicides have already betrayed the body.


Still-born, they don't always die,

But dazzled,  can't forget a drug so sweet

A face would look on and smile.

I am thirsty for compassion.

I can't bear this pain no more, the agony to kill myself

More and more people are being diagnosed with NET — just not soon enough

NET isn't understood very well and it can often be anywhere between five to seven years before a conclusive diagnosis is revealed.

More than half the people suffer from late detection of the condition, according to the Neuroendocrine Tumour Research Foundation ( NETRF). By that time, NET has normally spread to other organs in the body.


NETRF data also shows that every 7 in 100,000 people are afflicted and that number is on the rise. To raise awareness, the International Neuroendocrine Cancer Alliance (INCA) created the World NET Cancer Day, which occurs on November 10 every year.

Even so, it's not something that the common person will be familiar with until they're actually diagnosed. The tumours can occur almost anywhere in the body and don't differentiate between gender and age.

Voices to raise awareness of NET




Allan Rickman- best known for his role as Snape in Harry Potter — was experiencing stomach pain for some time. His doctor reassured him that it was just acid reflux, and was told to use antacids.

The following year, the pain returned, and he was referred for imaging tests. His scans showed a stage IV tumor that had spread to his liver. He lived for a little over a year longer, passing away soon after his 56th birthday in 2014.
The voices of late stars Alan Rickman, Aretha Franklin and more have been used to raise awareness for pancreatic cancer.


Pancreatic Cancer UK has marked Pancreatic Cancer Awareness Month (in November) by launching an appeal called Lost Voices: Help us break through the silence.

It aims to get donations to help with vital research into the deadliest common cancer.

Accompanied with an animation, the audio production includes the voices of the stars who have lost their lives to pancreatic cancer.


Voices used include Alan from Harry Potter, Sir John Hurt, Aretha Franklin, Patrick Swayze and Steve Jobs.

Other stars included are Olivia Williams and Ruby Wax as their lives were personally affected by the disease.

Medical advancements have vastly improved survival rates and time alive for cancers patients in general - but tumors of the pancreas remain some of the toughest to treat. 

Only 20 percent of pancreatic cancer patients survive for a year after diagnosis. Just seven percent make it five years.  

As other forms become more curable, pancreatic cancer is climbing the ranks of deadliest cancers, reaching number three this year, and set to soon become second only to lung cancer. 

The Queen of Soul had surgery, most likely for a pancreatic tumor in 2010, and went on to live for another eight years. 

Although hers was the less deadly form, an expert told Daily Mail Online why even a power house performer with every resource at her disposal like Franklin faces such tough odds with pancreatic cancer.

Sliding down the slope



 Am I sliding down the slopes?

As slippery slopes

I was swept off my feet a freak

For her glory and dazzle

Here I explain

Mum frequently pulled up to my feet during the interminable pain 

But I hit an iron table and a terracotta figurine fell on my brain and knee surgery site 

A warrior is born to battle and fight

Right knee swelled 

Thou knowest all my heart an' mind, Thou knowest, Lord, I fell.

Hurting toes 

Black and blue right big toe

Hurting elbow

Mo  is slow

My sprint was life going too slow

Engulfed in pain limping had

A hot water bath mum stood as I bathed in bleb of shampoo


There was a second severe fall too

On new year's eve I fell

But this I know full well

Gave a horrible dying yell


Mum came to rescue

She's clever and sensitive I tell you

Again hit the knee but the forehead 

As she from behind my arms and neck held.

Headaches of NET MET

 


Hope is something really tough and tireless

It's an addiction to break.

Even when I feel like hell, as pain wells up

Painkillers are no  use, what diff'rence would it make?

The problem is, in Hell there's no hope

All my life I had to grope

Now it feels like sliding down the slope

Now in darkness I am not fit to cope

Mum and me trudged through

I need PRRT full dose

 


Neuroendocrine neoplasms are tumors that arise from diffuse neuroendocrine system cells. They are most commonly found in the gastrointestinal tract, pancreas and lungs. Neuroendocrine neoplasms are fairly rare diseases, but their incidence and prevalence have increased substantially in recent decades. The majority of neuroendocrine tumors overexpress somatostatin receptors (SSTRs), which are targeted for imaging and treatment.


Somatostatin receptor (SSTR) alogues, which are used for imaging of neuroendocrine tumors (NETs), can also be used for therapy by replacing the imaging isotope with a therapeutic isotope. This general field is referred to as peptide receptor radionuclide therapy (PRRT). PRRT is not specific to NETs, but NET therapy is currently the most advanced and only U.S. Food and Drug Administration (FDA)-approved example of it. Furthermore, this combination of imaging and therapeutic isotopes with the same target is a good example of theranostics, which is a portmanteau word consisting of “therapy” and“diagnostics.” The term “theranostics” is fairly new, but the best example of it is one of the oldest isotopes, essentially originating 

the field of nuclear medicine, 131, which is used for simultaneous imaging and therapy.

Development of Peptide Receptor Radionuclide Therapy

NETs are well known for being heterogeneous. They can arise from a variety of different organs; can be benign or malignant; are well, moderately, or poorly differentiated, as in grade; have variable metastatic potential; and are either functional or nonfunctional (i.e., produce hormones and symptoms or do not) . Symptoms can markedly reduce quality of life and are an important consideration for therapy. NETs have a low incidence but a high prevalence given the lower mortality associated with these tumors.

One commonality among NETs is expression of the G protein–coupled SSTRs,of which there are five subtypes in humans. The human somatostatin molecule, composed of 14 amino acids, is a hormone used by neuroendocrine cells for neurotransmission and cell proliferation. An abbreviated version of the human somatostatin molecule, 

composed of eight amino acids, is octreotide (Sandostatin, Novartis). Octreotide was FDA-approved in 1988 for symptom control in patients with functional NETs (package 

insert, Novartis). However, it is also known to have antiproliferative effects and is practically used in that way for patients with lowor intermediate-grade metastatic inoperable NETs.



"Over the past two decades, SSTR-targeted imaging using radiolabeled somatostatin agonists followed by PRRT has been remarkably successful in managing neuroendocrine tumors," said Jingjing Zhang, MD, Ph.D., assistant professor in the Department of Diagnostic Radiology at the Yong Loo Lin School of Medicine at the National University of Singapore in Singapore. "However, potent SSTR antagonists—which only poorly internalize into tumor cells if at all—have surprisingly been shown to be even superior to agonists for such purposes."


To further investigate the role antagonists can play in treating neuroendocrine tumours researchers developed a study to determine the safety, biodistribution and efficiency of a new type of SSTR antagonist, 177Lu-DOTA-LM3. Fifty-one patients with progressive, heavily pretreated neuroendocrine neoplasms underwent PRRT with 177Lu-DOTA-LM3. Treatment-related adverse events were graded for all participants, and dosimetry was performed for 11 patients.


177Lu-DOTA-LM3 was administered without severe adverse effects and was well tolerated by most patients. Disease control was reached in 40 out of 47 patients (85 percent) who were monitored after 177Lu-DOTA-LM3 therapy. Two patients achieved complete remission by the European Organization for Research and Treatment of Cancer criteria.


Of note, the uptake and dosimetry of the antagonist 177Lu-DOTA-LM3 were compared with those of the commonly used SSTR agonist 177Lu-DOTATOC in patients undergoing treatment on the same dosimetry protocol. 177Lu-DOTA-LM3 demonstrated higher uptake and a longer effective half-life in tumor lesions, resulting in higher tumor radiation doses than for agonist 177Lu-DOTATOC.


"These encouraging findings demonstrate the feasibility and superiority of SSTR antagonist 177Lu-DOTA-LM3 as compared to SSTR agonists. Furthermore, antagonist PRRT can be performed under concurrent treatment with somatostatin analogs without the need for interrupting these medications. This is especially important for patients suffering from carcinoid syndrome or even carcinoid crisis,". "The results are very encouraging for theranostic applications of SSTR antagonists to further improve outcomes in patients with neuroendocrine neoplasms in the future."

Hence the growth of new blood vessels or angiogenesis is not so much a disease as a phenomenon, the result of a basic evolutionary compromise. As a body lives and grows, its cells are constantly dividing, copying their DNA — this vast genetic library — and bequeathing it to the daughter cells. Thus recovery or cure is a forlorn hope as this growth phenomenon exploits evolution, Darwinism. All we can do is increase longevity, thereby attempting to understand the phenomenology behind VHL and increase the efficacy of modern medical techniques. Increasing the outcome of the average lifetime has to be the motto not a baseless idea of eradicating.

Von Hippel-Lindau disease is a neurocutaneous syndrome. A neurocutaneous syndrome causes problems that affect the brain, spine, and nerves (neuro), and the skin (cutaneous).

In Von Hippel-Lindau disease, tumors most commonly develop in the brain and retina of the eyes. These tumors, called angiomas, consist of blood vessels. Other types of tumors develop in other organs and include tumors in the adrenal glands (pheochromocytomas) and cysts in the kidneys, liver, or pancreas. As people with the disorder age, the risk of developing kidney and pancreatic cancer increases. vHL patients battle a series of tumours. vHL gene is involved in many forms of cancer.
